In your last Diving Deeper we invited you to chose your sponsor and write a letter to them.

In this one, we need you to work with your parents to retrieve a copy of your sponsor confirmation certificate.

This document can be retrieved by asking the sponsor to call or email the church they were confirmed at.

Please submit these documents by January 20th

Instructions

Here are some things to consider when choosing your Sponsor:

They must be ...

At least sixteen (16) years of age
baptized and confirmed Catholic
A practicing Catholic
A good model of Christian living who can help you grow in their experience of God through the practice of their faith
A good listener in whom you can confide
Interested and involved in your faith experience
Your sponsor cannot be your parent

Please submit a hard copy of the confirmation certificate in person.

This must be an official document from the church with a seal. Not a certificate presented at the day of their confirmation.

Please only submit this form when you have obtained the certificate of confirmation of the sponsor chosen.

You will receive an email receipt that will acknowledge a successful submission.

*A scan of the Confirmation Certificate saved in PDF format is the most convenient type of file. The writing should be clear and legible.

Please hand in a hard copy at an Upper Room session.
